What is the CareEngine System?

Who maintains the CareEngine System?
At the foundation of all of our clinical solutions is the ActiveHealth® Clinical Development Center, which is staffed by a large team of board-certified physicians, pharmacists and registered nurses. Responsible for developing and maintaining the current, robust set of rules that make up our patented clinical decision support technology, the team meets daily to review and discuss evidence-based literature. They update existing Care Considerations and develop new Care Considerations on an ongoing basis.

What is Active Disease Management?
Active Disease Management is a voluntary program provided by your employer as part of your health plan. The program is designed to help individuals with chronic health conditions understand their condition and work with their doctor(s) to better manage their care. Active Disease Management supports the relationship between the doctor and the patient by providing information regarding treatment options, which includes Care Considerations. Active Disease Management is offered to you as part of your health benefit plan and is designed to help you better manage your health and take active steps to work with your doctor to improve your care.

Is Active Disease Management a traditional disease management program?
Yes. Just like traditional disease management programs, Active Disease Management is staffed by experienced registered nurses and is accredited by NCQA and URAC. Our nurses educate, empower and motivate members to better understand their conditions and take actions to improve their health. Active Disease Management differentiates itself by actively engaging physicians and members, communicating targeted, patient-specific information, and addressing multiple conditions in one care plan. What is a Care Consideration?
A Care Consideration is a personalized, specific piece of information about a potential issue regarding your health care. We encourage you to discuss any Care Consideration you receive with your doctor.

How did you get my name and personal health information?
As part of the quality initiatives included in your health plan, your medical information is sent to us so we can run it through a sophisticated process to identify care concerns and areas in which your health care may be improved.

Will you share my personal health information with anyone?
We maintain the confidentiality of your personal health information in compliance with the Health Insurance Portability and Accountability Act (HIPAA) privacy regulations. Your information is shared only as permitted or required by law.

Will I be charged extra, at any time, for participating in this program?
No. This program is offered as part of your health benefits and comes at no extra cost to you.

If I choose to participate in the program, how often will I be contacted?
If you choose to participate in this program, you will be contacted about four times a year, or more frequently if we have specific information or follow-up questions to discuss.

Can I talk to my doctor before agreeing to participate in this program or talk to an Active Disease Management Nurse?
Absolutely. Your doctor may have already received information about our program and may be able to help you decide whether or not you would like to participate. After you've spoken to your doctor, please feel free to call us, or we can contact you.

If I place a call to a nurse, how much time will it take?
We just need a few minutes of your time to get you started. We are flexible and will work with you according to your availability.

Why wouldn't my doctor know about any health issues identified by Active Disease Management?
Physicians don't always have access to all components of a patient's clinical profile, especially if the patient is seeing more than one doctor. The clinical data provided by your health plan allows us to identify if you may be at risk for certain medical conditions or may benefit from certain treatments or preventive screenings and to communicate this information to you and your doctor.
In addition, by joining the program and speaking with our Active Disease Management Nurse, he or she can provide you with a list of questions to take to your doctor's visit.

Is talking to an Active Disease Management Nurse a substitute for a doctor's visit?
No – in fact, quite the contrary. Our Active Disease Management Nurses will serve as your personal health coach and provide you with important health information for you to share with your doctor that can enhance the relationship you have with your doctor.

Will an Active Disease Management Nurse make treatment recommendations?
No. Our Active Disease Management Nurses identify potential health issues and provide information about your health care and may provide treatment options for you and your doctor to consider.
