Educating and coaching.
Customized health assessments are available for 35 adult conditions and 6 pediatric conditions. Nurses educate members on self management techniques so that they can take better control of their conditions. They provide coaching on ways to improve their health, including lifestyle factors and behavioral changes. In addition, they review warning signs for all conditions and discuss appropriate actions to take if they occur.

Clinical foundation of Active Disease Management.
At the foundation of the Active Disease ManagementSM program is the ActiveHealth® Clinical Development Center, made up of a large team of full-time board-certified physicians, pharmacists and registered nurses. This clinical team develops new CSID matrices, care plans to guide our Active Disease Management program, and Care Considerations based on nationally-accepted evidence-based practice guidelines and literature. Care Considerations and matrices are updated three times annually, along with a formal annual review of all programs and clinical guidelines.
Our clinical team meets daily to discuss new issues and developments in the medical field. Rigorous reviews of the following evidence-based sources are conducted:
- publications and position statements of major medical organizations
and peer-reviewed journals,
- standard medical textbooks,
- discussion with subspecialty panelists,
- and other sources of evidence-based clinical data.
