ActivePHR The ActivePHR® program is a secure online personal health record powered by the CareEngine System that provides members with an evolving, comprehensive picture of their health. ActivePHR® comes pre-populated with data generated from the CareEngine System. The integrated health risk assessment (HRA) enables members to learn about risks, goals and ways to improve their health. The HRA also provides an easy way for members to enter personal health information that only they know, such as smoking status, allergies, family history, over-the-counter medications, and herbal supplements. Upon completion of the HRA, the member-entered data is combined with all other information ActiveHealth has for that member and is sent to the CareEngine System in real-time. Members may receive an instantaneous clinical alert if CareEngine System identifies a specific opportunity for better care. Their physician may also be notified. Active Lifestyle CoachingSM Active Lifestyle CoachingSM is our dedicated lifestyle coaching program that complements the wellness components that are integral to all of our solutions. The purpose of the program is to identify the at-risk members in a population and impact them before they develop costly chronic conditions or other health issues. The program empowers individuals to quit tobacco, achieve their target weight, adopt healthier diets, develop healthy exercise habits and cope with stress. Health coaches establish an ongoing, telephonic relationship with each member and use various methods to encourage members to incorporate healthy behaviors and to achieve their lifestyle goals.

Active Benefit Design The Active Benefit DesignSM program is a value-based insurance design (VBID) solution based on a member's personal profile and evidence-based sources. The program enables design of a formulary and co-pay structure that provides those individual members who derive the most clinical benefit from specific drugs with the most financial incentive to use those drugs. This program motivates members to take their required drugs and ensures contraindicated drugs are not inadvertently administered.

ActiveAdvocate The ActiveAdvocateSM program provides health plans, physicians, and members with one nurse who has the knowledge, resources and evidence-based recommendations to help members make critical medical decisions. Members have one nurse to call to find answers to questions, understand their options, learn the right questions to ask, and identify resources for themselves and their family.

Active Maternity Management The Active Maternity ManagementSM program is designed to help reduce the risks and costs associated with premature birth and complications of pregnancy. A Nurse Care Manager conducts a full health and risk assessment of an eligible member, and provides educational material and support throughout the pregnancy.

ActiveUM/CM The ActiveUM/CMSM program offers traditional utilization and case management services. Utilization management services (UM) include ensuring effective identification and medical management of individual and general medical utilization issues. Case management (CM) services include coordination of care for complex and high-cost cases.
