ActiveHealth PHR ActiveHealth PHR is a secure, online personal health record powered by the CareEngine® System. ActiveHealth PHR provides a dynamic analytic capability that informs members and their physicians about targeted and actionable opportunities to improve healthcare quality and lower costs. Pre-populated with medical, lab and pharmacy claims, the PHR also allows members to enter personal health information not available from claims. As new data is received, it is scanned by the CareEngine. Value-Based Formulary ActiveHealth's Value-Based Formulary is a customized formulary based on a member's personal profile and evidence-based sources. The program enables design of a formulary and co-pay structure that provides those individual members who derive the most clinical benefit from specific drugs with the most financial incentive to use those drugs. This program motivates members to take their required drugs and ensures contraindicated drugs are not inadvertently administered.

Health Advocate ActiveHealth's Health Advocate provides health plans, physicians, and members with one nurse who has the knowledge, resources and evidence-based recommendations to help members make critical medical decisions. Members have one nurse to call to find answers to questions, understand their options, learn the right questions to ask, and identify resources for themselves and their family.

Maternity Management ActiveHealth's Maternity Management program is designed to help reduce the risks and costs associated with premature birth and complications of pregnancy. A Nurse Care Manager conducts a full health and risk assessment of an eligible member, and provides educational material and support throughout the pregnancy.

Utilization Management/Case Management (UM/CM) ActiveHealth's Utilization and Case Management (UM/CM) unit has been providing utilization and case management services since the 1980s. Utilization Management services include ensuring effective identification and medical management of individual and general medical utilization issues. Case Management services include coordination of care for complex and high-cost cases.
