Communicating actionable, timely information.
Our programs do not replace physician visits, give medical advice or deliver clinical care. Rather they communicate timely, actionable clinical information to both physicians and their patients. We notify physicians by phone when there is an urgent issue regarding one of their patients, and by fax or mail for less urgent issues. Similar information is communicated to members, and they are encouraged to discuss the issue with their physician. Members who are enrolled in our Informed Care Management (ICM) program are educated on target health goals for their conditions, made aware of warning signs and learn about what to do if they occur.
